60 an hour 12 days ago 12 days ago 12 days ago Purpose of Role The Intake Assessor ensures the smooth admission and on-boarding of new clients into Transformations Mending Fences by ensuring all the required departments are notified of the client's arrival.
Assisting the assimilation of the client into Transformations Mending Fences program Core
Responsibilities:
Supervise the client intake process.
Responsible for quality control of the intake process, revise each new chart and making necessary corrections.
Train new employees on the Medical Records system and the intake process.
Coordinate client arrivals with outside facilities and lead behavioral technicians.
Work with medical staff to prepare for nursing assessments, checking new intake medications and ensuring they get to medical staff.
Facilitate new client assessments.
May Facilitate support groups and complete all proper documentation.
Responsible for quality control of client personal belongings, log personals into Medical Records system, sign off on personals (double checking), sort new client personal belongings in the appropriate places, checking new client personal belongings upon discharge and check and balance new client cash records.
Quality control of weekend intakes and personal items.
Check and balance new client cash records through the weekend.
Audit new charts, pull clients' charts and correct forms if necessary.
Create new admit forms and folders for all incoming intakes, making sure appropriate parties signatures are obtained; they are complete and inserted into binder.
Ensure urine analysis forms are signed and completed properly, print out new admit face sheets and pregnancy tests to go with u/a.
Pull personals for all weekly activities Manages and accounts for all client valuables.
Pull personals for discharging clients and verifying all items are accounted for, tracking missing items, ensure they get to the appropriate parties (Lead Techs and Discharge Coordinators).
Conduct searches and urine analysis on incoming admits.
Tag baggage of new admits.
Assist clients looking for mail, phone numbers, money, cigarette cards, etc.
Train behavioral techs on intakes and assessments, reviewing errors and corrections.
Adhere to all company policies and safety rules, guidelines & procedures.
Professional Experience, Essential Skills, and Knowledge 1 year Previous experience in the Substance Abuse Field All employees may be required to work before, during and/or immediately after a natural or man-made disaster Is knowledgeable of Client rights and treats all clients with dignity and respect in a consistent manner; adheres to federal confidentiality standards (HIPAA) Ability to prioritize workload and work independently, be thorough and communicate effectively with staff, management, and clients Knowledge of State & Federal Statutes Regarding Patient Confidentiality laws Must be reliable and consistent with attendance and punctuality to work Salary Range $22.
